How to get application
Controlling directives/guidelines for program applicants Patients must have no insurance coverage for prescription required. Must be treated out-patient and treated on an ongoing basis. Patients must also meet program financial guidelines.
Beginning course of action to obtain meds Doctor's office calls for an application to be faxed. A case number is required for application to be valid. Completed application may be mailed or faxed.
Doctor/provider's responsibilities of action Doctor fills out section on application. After application is completed, doctor calls company to get the patient's unique case number and instructions on completing application process.
Patient's responsibilities of action Patient or patient advocate fills out section with financial and insurance information.
Manner of distribution Medication is sent to the doctor's office.
Amount distributed Oncology medications - 2 month supply. Virology medications - 3 month supply. Hydrea and Cytoxan tablets - 6 month supply.
