Controlling directives/guidelines for program applicants Patient firstly must be in the TogetherRX program, a discount prescription medication program for people on Medicaid or Social Security due to disability. For more information on this program call 800-865-7211 or go to www.together-rx.com. Program covers medication made by Janssen, McNeil, and Ortho-McNeil.
Beginning course of action to obtain meds Patient calls to start process. The majority of the application is completed over phone with program personnel. Once initially complete, application is faxed or mailed to patient.
Doctor/provider's responsibilities of action Provides prescriptions to the patient.
Patient's responsibilities of action Confirms information on application to be correct, signs it and attaches proof of income before sending it back to program.
