How to get application Call program
Controlling directives/guidelines for program applicants US residency required, have household income not more than 350% of the federal poverty level and have no prescription insurance. Program requests an 'Advocate' be contact person for patient during entire process. Advocate may be any healthcare worker involved in patient's care. (e.g. Physician, nurse, social worker, satisfy requirements)
Beginning course of action to obtain meds Advocate calls for an application to be faxed or mailed with patient ID number (Enrollment process may instead be started online at commitmenttoaccess.gsk.com). Once application completed, advocate calls program to start enrollment process. Completed application must also be mailed to program.
Doctor/provider's responsibilities of action Completes application section. If program accepts patient application then fax prescription to 1-800-750-9832 ( indicate ""Attention: Direct Ship Specialist."").
Patient's responsibilities of action Completes section of application about monthly gross income and insurance and attaches proof of income. Advocate assists and coordinates.
Manner of distribution Program sends medicine to doctor's office usually within a week of approval.
Amount distributed 30 day supply
How to begin refill process Advocate calls 5 to 7 business days before the medication runs out to order next 30-day supply. New application required of patient and advocate each year.
