How to get application request applicationes
Controlling directives/guidelines for program applicants Co-pay for each shipment ranging from $5.00-$35.00. Patient must be US resident with MS and meet financial guidelines-call program. Support program - very developed including registered nurse counselors available 24 hours a day/seven days a week. Training also provided. After one year a whole new application is needed.
